이명박 대통령이 22일 "정부의 복지 예산은 매년 늘어나고 있으며, 내년 복지 예산은 역대 최대"라며 "우리가 복지국가라고 해도 과언이 아닐 정도의 수준"이라고 말했다고 한다. 최근 날치기 예산 통과 과정에서 각종 서민예산이 삭감된 데 대한 시민들의 비판과 야권의 공세가 거센 점을 의식한 발언으로 보인다.
하지만 결론부터 말하면 이 대통령의 이날 발언은 ‘과대망상이라고 해도 과언이 아닐 정도의 수준’이다. 왜 그럴까. 이를 따져보기 위해 우선 12대 주요 분야별로 2011년 예산이 어떻게 배분되는지 <도표1>을 참고로 살펴보자. 분야별로 보면, 보건복지 86.3조원, 일반공공행정 53.2조원, 교육 41.3조원, 국방 31.3조원, SOC 24.3조원 등의 순으로 나타나고 있다.

현 정부의 전반적 기조를 보기 위해 경제위기 이전에 편성된 예산이자 현 정부 출범 첫 해인 2008년의 예산 대비 2011년 예산안의 분야별 증가율을 살펴보자. 우선, SOC 예산은2008년 대비로는 24.0%나 늘어 전체 총지출 증가율 17.8%보다 훨씬 더 많이 늘어났다. 거꾸로 2010년 대비 7.8% 늘어나 평균 증가율보다 높은 교육 예산은 2008년 대비로는 16.0% 증가에 그쳐 평균 총지출 증가율을 밑돌고 있다. 물론 R&D와 보건복지 예산의 증가율이 여전히 크게 나타나기는 한다.
어쨌든 겉보기에 "정부의 복지 예산은 매년 늘어나고 있으며, 내년 복지 예산은 역대 최대"라는 이 대통령의 말이 틀린 말은 아닌 것처럼 보인다. 같은 맥락에서 친기업 신문을 자처하는 일부 언론들도 정부의 ‘선심성 복지지출’ 증가를 우려하는 기사를 쏟아내고 있다. 그런데 가만히 생각해보자. 위에서 본 것처럼 경제위기 이후 급격히 늘렸다가 내년 예산안에서 줄어든 SOC예산을 빼고 나면 매년 늘어나고, 역대 최대가 아닌 예산 항목이 어디 있는가. 너무나 당연한 사실을 대단한 의미가 있다는 듯이 너스레 떠는 것은 황당하기 짝이 없다.
더구나 속사정을 알고 보면 현실은 오히려 이대통령 발언과 반대에 가깝다. 우선, 정부가 보건복지 예산으로 잡은 항목들 가운데 상당 부분은 실제로는 보건복지 예산이 아니다. 예를 들어, 국토해양부 소관 기금 중 하나인 국민주택기금의 2011년 지출액은 2010년 대비 1.2조원 늘어난 17.8조원에 이른다. 전체 보건복지예산의 20.6%에 이르는 금액이 보건복지 예산으로 잡혀 있다. 하지만 기금의 성격을 뜯어보면 보건복지 예산이라기보다는 토건 SOC 예산에 오히려 가깝다. 국민주택기금 지출액의 약 65% 가량이 각종 주택 건설사업에 들어가는 돈이기 때문이다.
특히 2011년의 경우 기금 지출액의 53%가 넘는 9.5조원이 보금자리 주택사업에 지원된다. 그런데 보금자리주택사업의 3분의 2 가량은 서민용 공공임대/전세 주택이 아닌 공공분양 물량이다. 실제로
또한 나머지 35% 가량을 차지하는 주택구입 및 전세 융자금 지원액도 보건복지 예산으로 잡기 어렵다. 이들 융자액은 일정한 시점에 원본에 이자까지 덧붙여 기금으로 회수하는 것이므로 재정지출액이 아니기 때문이다. 이 밖에도 보건복지 예산으로 분류된 예산 가운데 상당 부분은 각종 복지시설 건립비 등 사실상 토건 사업 예산인 경우가 수두룩하다. 이처럼 성격상 보건복지 예산으로 잡아서는 안 될 예산을 복지예산으로 산입해 마치 ‘복지대국’인 것처럼 부풀리고 있는 것이다. 하지만 정부는 자신들의 분식행위가 드러날까 봐 그런지 복지 예산의 항목별 소상한 내용을 국민들에게 공개하지 않고 있다.
또 하나 고려해야 할 사항은 보건복지 예산은 저출산 고령화 추세가 급속히 진행되고 있어 재정수요가 급증하는 분야라는 점이다. 이미 과잉 투자돼 재정수요가 급감하고 있는 SOC예산과는 정반대 흐름을 나타내고 있다.
예를 들어, 기본적인 사회보장 및 복지 제도에 따라 의무적인 지출액만 따져도 계속 커질 수밖에 없는 영역이다. 매년 최저생계비가 인상되면 그에 준해 기초생활수급 대상자 수와 수급액이 늘어날 수밖에 없다. 또한 2010년대에는 베이비붐 세대의 본격적인 은퇴가 본격화하면서 국민연금을 내던 사람들이 이제는 연금을 타쓰는 사람으로 전환되게 된다. 이런 식의 의무적인 자연 증가분만으로도 매년 복지예산은 증가할 수밖에 없다.
이는 우리보다 앞서 고령화가 진행된 나라를 보면 쉽게 알 수 있다. 미국의 경우 정부 경상지출 대비 사회보장비 지출 비중이 1990년 30% 수준에서 2009년 41% 수준까지 늘어났다. 일본의 경우에도 1990년 16.5% 수준에서 2006년에는 25%를 넘어섰다. 서구 유럽에 비해 복지수준이 상대적으로 낮은 미국과 일본조차도 고령화 등이 진전됨에 따라 사회보장지출 비중은 꾸준히 늘어날 수밖에 없다. 더구나 현재 한국은 세계의 대표적인 고령화 국가인 일본보다 더 빠른 고령화 추세를 나타내고 있다. 이런 상황에서 보건복지지출 비중은 가만히 있어도 매년 역대 최대가 될 공산이 매우 커진다. 그렇다고 해서 미국이나 일본 등의 언론들이 이런 현상을 두고 매년 ‘복지예산 역대 최대’라며 복지예산을 줄이라는 식으로 보도했는지는 의문이다.
실제로 당장 내년 예산안 가운데 공적연금 지원액 2.2조원, 보훈보상금 0.7조, 건강보험 지원금 0.3조, 의료급여 0.2조, 기초노령연금 0.1조, 노인요양보험 0.1조 등 3.6조원이 의무적인 지출 증가분에 해당한다. 또한 주택부문 증가분 1.3조원도 사실상 융자금 성격의 돈이어서 복지지출로 보기 어렵다. 이에 더해 앞서 부당하게 복지예산으로 산입된 국민주택기금의 증가분 1.2조원을 합치면 약 4.8조원이다. 이것만으로도 2010년 대비 보건복지 예산 증가액 5.1조원과 맞먹어 버린다. 의무적 지출이나 사실상 복지예산이 아닌 항목을 빼면 실제로는 보건복지 예산이 거의 증가하지 않은 셈이다.
더구나 2011년 물가 상승률을 약 3%로 잡는다면 실질 가치로는 3% 가량 보건복지 예산이 줄어든다는 계산이 나온다. 또 GDP나 전체 예산규모 대비 다른 부문 예산안이 늘어난다는 것을 감안하면 정부가 자기 재량으로 늘리는 보건복지 예산은 오히려 뒷걸음질치고 있다는 뜻이다.
사정이 이렇다 보니 기존 취약계층이나 저소득계층에 대한 복지지원액은 오히려 줄고 있다. 이 문제에 대해 꾸준히 추적하고 있는 참여연대에 따르면 복지예산 가운데 취약계층을 위한 예산 비중은 2006년 18.7%였으나 2008년 15.6%, 2009년 12.4%, 2010년 11.8%로 줄었고 2011년 예산안에서는 11.5%로 떨어졌다. 특히 건강보험 본인부담금과 보험료 등을 면제받는 의료급여 수급자가 크게 줄어들었다. 2007년 197만 8000명(인구 대비 4.1%)이던 의료급여 대상자는 2010년에는 174만5000명(인구 대비 3.6%)로 줄었고 2011년에는 172만5000명(3.5%)로 줄어들 것으로 보인다고 한다.
4인가족 기준으로 월소득 186만7435원 미만 가구를 나타내는 기초생활수급대상자 역시 2010년 163만2000명에서 2011년에는 160만5000명으로 줄어든다. 이들을 위한 생계급여 예산도 올해의 2조4491억원에서 2011년 예산안에서는 2조4459억원으로 32억여원 줄어든다.
이에 더해 국회 예산 통과 과정에서 삭감된 복지 예산이 적지 않다. 2009년 542억원, 2010년 203억 원이 배정됐던 방학중 결식아동 예산과 영유아 예방접종비 194억원이 전액 삭감된 것이 대표적 사례다.
이처럼 무자비한 서민예산 삭감이 현실에서는 어떤 충격을 미칠까. 필자는 2008년 말 당시 사회복지사로 일하던 필자의 아내와 함께 며칠간 경기도 고양시의 기초생활대상자들을 돌아본 적이 있다. 아내의 얘기를 듣고 어느 정도 알고는 있었지만, 한국의 열악한 복지 현실에 마음이 찢어지는 듯 했다. 생활도우미가 절실히 필요하지만 도움을 받지 못해 변도 처리하지 못하고 있는 장애인, 매일 끼니를 라면으로 때우고 전기요와 홑이불 몇 개에 의지해 겨울을 나던 60대 노인, 컨테이너 박스에서 노환에 시달리며 한 달 생활비 30만원으로 겨우 살아가던 독거노인, 차상위 계층에 대한 건강보험 지원이 끊기면서 약값 부담 때문에 발을 동동 구르던 할머니...
그런데 아내와 동료 사회복지사 한 명의 급료를 포함해 80여 가구를 대상으로 하는 복지사업에 배정된 1년 예산은 겨우 1억원. 아내는 예산이 조금만 더 있어도 정말 많은 일을 할 수 있겠다며 안타까워했다. 그런데 그 해 말 정부는 경제위기 극복과 서민경기 부양을 명목으로 각종 토건사업을 대대적으로 벌이고 조기 예산 집행에 나섰다. 당시 여당 소속 시장이 있던 고양시도 비슷하게 움직였다. 그러면서 제 처가 담당하던 거점센터에 지원하기로 했던 예산은 당초보다 3000만원 깎이고 말았다.
중앙정부는 국민들의 반대 여론이 높은 4대강 사업에 수십조원을 투입하고, 고양시는 지금도 가동률이 50%에 불과한 킨텍스 옆에 제 2전시장을 짓는다며 3500억원을 쓴다. 고양시 1년 전체 사회복지예산의 1.5배에 이르는 돈이다. 상론하기는 어렵지만, 턴키사업으로 진행되는 그 건설사업비 가운데 1000억원은 건설업체에 그냥 퍼주는 돈이나 다름 없다. 도대체 한 달에 단돈 몇 만원이 아쉬워 최소한의 인간적 삶도 못 누리는 이웃들을 방치하면서 이게 뭣 하는 짓인가.
이처럼 열악한 대한민국 복지 현실은 OECD 국가간 비교에서 명확히 드러난다. GDP 대비 한국의 공적사회복지지출은 8% 수준으로 경제협력개발기구(OECD) 국가 평균 21%의 3분의 1수준을 조금 넘는다. 이런 상황에서 이 대통령이 ‘복지국가로 진입하고 있다’고 말하고, 윤증현 기획재정부장관이 ‘복지를 즐긴다’고 표현하며,
물론 일부 정책 기획과 집행 과정의 문제로 복지 예산 가운데도 문제 소지가 있는 정책은 분명히 있다. 하지만 그것은 복지예산이 전반적으로 과도한 것과는 무관하게 정부 정책의 기획 및 집행과정상의 문제, 그리고 관료시스템 상의 문제인 경우가 더 많다. 그런 문제는 굳이 복지가 아니라 다른 예산 분야에서도 새고 샜다.
물론 복지라는 것이 무조건 돈을 많이 쓴다고 되는 것도 아니고, 국민들의 세 부담이나 생산경제에 대한 위축효과 등을 생각하지 않고 무작정 추진하는 것은 문제다. 예를 들어, 세계 최저수준의 저출산 문제를 해결하는 것도 무작정 예산을 퍼붓는다고 해결되는 것이 아니다. 높은 집값과 ‘승자독식구조’에 가까운 사교육비 경쟁, 양질의 일자리 부족으로 인한 청년층의 만혼화 현상 등 건전하고 지속가능한 사회경제적 구조를 만드는 것이 근본적인 해법이지 저출산을 강요(?)하는 구조는 그대로 둔 채 예산을 퍼부어봤자 막대한 재원만 낭비하게 될 가능성이 높다. 그러지 말고 우리 연구소가 설명한 것처럼 국민연금 등 공적사업자가 나서 대규모 공공임대/전세주택을 공급하면 재정을 낭비하지 않으면서도 저출산 문제와 노후 문제를 동시에 해소할 수 있게 될 것이다.
하지만 그럼에도 불구하고 현재 국내 복지수준이 상당히 열악하다는 점과 향후 저출산 고령화의 충격이 본격화됨에 따라 복지수요가 급증할 수밖에 없다는 점에서 지금부터라도 전략적으로 일정 수준의 사회안전망과 복지지원체계를 단계적으로 준비해가지 않으면 안 된다. 그 같은 재원은 자산경제 부문에 대한 과세 확충과 지하경제의 투명성 강화 등을 통한 조세구조 개혁과 불요불급한 토건사업 억제 등 세출 구조조정을 통해 확보하는 한편 체계적인 정부시스템 개혁을 통해 정책 기획 및 집행의 효율성을 높여가야 한다. 필자가 출간한 새 책 <프리라이더: 대한민국 세금의 비밀>을 집필하면서 계산해본 결과로는 세입 구조개혁을 통해 50조원, 세출 구조조정을 통해 50조원 등 모두 100조원의 예산을 추가로 확보할 수 있는 것으로 나타난다. 이렇게 확보한 예산으로 건전하고 지속가능한 사회경제구조를 만들면서 우리의 열악한 사회안전망과 복지 인프라를 확충하는 작업이 필요하다. 특히 저출산 고령화 충격이 본격화하기 전에 지금부터 전략적으로 복지 인프라 구축해가는 작업 서둘러야 한다.
그런데 저출산고령화 충격이 눈 앞에 닥쳐 와있는 상황에서 현 정부는 기본적인 사회안전망을 구축하기는커녕 기존의 매우 부실한 사회안전망과 열악한 복지지원체계마저 해체하면서 자신들의 사리사욕을 채울 각종 개발예산들을 남발하고 있다. MB예산 (4대강사업 예산과 보금자리주택사업 예산 등)과 형님예산 등이 대표적 사례다. 그러면서도 한편에서는 자신들의 사리사욕을 얼버무리고 각종 복지예산을 삭감한 것을 호도하기 위해 대통령은 ‘복지 국가’라고 부르짖고 서울시장은 ‘망국적 복지 포퓰리즘’이라는 억지 핑계를 대며 이념공방으로 몰고 가려는 것이다. 공공사회복지지출 OECD 3분의 1만 쓰고도 말 몇 마디면 복지국가가 되는 것이라면 왜 유럽국가들은 수십 년에 걸쳐 그렇게 어렵게 복지국가 수준에 도달했겠는가? 실제로는 온갖 복지예산들을 마구 깎아대면서도 마치 복지예산이 넘쳐나는 나라인 것처럼 대통령과 서울시장이 국민들을 기만하고 있다. 도대체 대통령과 서울시장은 어느 나라에 사는 사람들인가?
